Onsite Work Requirement: This position is primarily onsite and requires regular in-person presence to support patients, families, alumni programming, discharge planning, tours, family events, and collaboration with clinical and operational teams. The ideal schedule is three days per week in Seattle and two days per week in Bellevue, with Seattle serving as the primary office location and taking priority for onsite attendance. Flexibility may be required based on business and operational needs. Due to the highly collaborative and relationship-focused nature of the role, candidates must be able to work onsite consistently and actively participate in facility and community-based activities and events. This position is not eligible for a fully remote work arrangement. What You'll Be Doing: The Alumni and Family Coordinator supports alumni and family engagement efforts across their assigned region, while being based onsite at an integrated care facility. This role partners with patients and families throughout the continuum of care—from admission through discharge and into sustained recovery. Working closely with clinical, outreach, and operations teams, the Coordinator helps facilitate aftercare planning, supports family engagement efforts, and strengthens connections with alumni and community resources. As an onsite presence, the Coordinator ensures these services are integrated into the patient experience while extending support across the broader region. ## ESSENTIAL DUTIES AND RESPONSIBILITIES: - Provides critical consultation in the patient transition and discharge planning process; coordinates with outreach team as needed to stay up to date on referral source activity to ensure relationships are fostered and maintained, coordinates in providing strategic referrals for Lower Level of Care (LLOC) discharges, supports referral relationships as needed. - Leads onsite aftercare and discharge planning groups to educate patients about the importance of aftercare; connects them to community resources and outlines alumni services and offerings. - Serves as the subject matter expert in the full patient journey; able to speak to or support needs across admission to discharge at all levels of care for both patients and caregivers. - Coordinates and supports on-site family programming and events. - Facilitates weekly virtual alumni and family support groups & provides appropriate follow up for referrals to Higher Level of Care (HLOC) as needed. Facilitates annual in person alumni event. Supports aftercare needs for former patients & families through follow-up phone calls/emails. - Facilitates tours for prospective patients and families. - Coordinates alumni engagements and speaking opportunities.
