You’ll use your clinical expertise to assess needs and risk, provide brief interventions and crisis support, and connect clients with the right care and resources. In this fast-paced, high-volume role, you’ll help remove barriers to care through personalized support, making it easier for clients to take the next step toward connecting to care. You must be able to work the following shift(s): Tuesday - Saturday 9:30am - 6pm PST (10:30am - 7pm MT / 11:30am - 8pm CT / 12:30pm - 9pm ET) Sunday - Thursday 9:30am - 6pm PT (10:30am - 7pm MT / 11:30am - 8pm CT / 12:30pm - 9pm ET) Weekends include a 10% shift pay differential! This role's internal title is: Clinical Care Navigator. The position can be carried out fully remote/virtually. Remote candidates must be physically located within the United States. ## Responsibilities - Provide compassionate, timely, evidence-informed clinical support to clients experiencing a range of emotional, behavioral, and situational concerns, including crisis and elevated-risk situations. - Conduct risk assessments and safety planning when indicated, and use appropriate de-escalation, rapport building, and evidence based coping interventions to provide clients with in the moment support. - Provide brief support and resources across a broad range of presenting concerns, including mental health, substance use, relationship and family concerns, workplace stress, grief, and other life challenges. - Support clients with connecting to Lyra resources and care options aligned with their clinical needs and preferences. - Utilize Lyra’s technology platforms, clinical resources, and provider network to facilitate efficient access to appropriate services. - Provide assisted booking and other care-navigation support to help clients successfully connect with recommended providers and services. - Conduct follow-up outreach when indicated to support continuity of care, reassess risk, and address barriers to connecting to ongoing care. - Respond effectively to a high-volume, high acuity inbound call queue, while maintaining excellent clinical quality, appropriate and timely documentation, client engagement, and adherence to established internal protocols and procedures. - Maintain accurate, timely, and clinically appropriate documentation of client interactions, assessments, interventions, referrals, and follow-up. - Collaborate with relevant Lyra teams to support safe, effective, and seamless client care. ## Qualifications - Master’s degree in clinical social work or equivalent; LCSW or equivalent independent clinical licensure is required (e.g. LMFT, LPC, LPCC, etc) or a Registered Nurse (RN). We’re unable to provide supervised hours toward licensure. Candidates without a state license to deliver mental health services, independently, will not be considered (with the exception of the RN license) - RNs must have two years of specific behavioral health experience including risk assessment and safety planning. - All other licensures must have two years post license experience (EAP/clinical call center experience preferred) - Significant experience managing high-acuity clinical situations (e.g., active suicidality/homicidality, NSSIB, mandated reporting) - Must be Bilingual, Spanish-speaking - Must be a full time resident of the United States; work cannot be completed from outside the United States.
