The Opportunity: Abby Care is at an exciting inflection point - since 2022 we have grown to support family caregivers in 8 markets nationally, and we're thrilled to launch Abby Care in the greater Phoenix market, and as a result, build our founding team. Critical to our success is hiring for a founding Care Partnerships Manager to help us grow our presence and deliver on our mission - helping families find Abby Care. As our Care Partnerships Manager, you'll own key relationships from case management leaders and supervisors at our health plan partner, to individual case managers. Your role is to make referring to Abby Care the simple and obvious choice, ensuring a case manager never has to chase us for an update. Initially, you'll also support provider relationships ensuring they are educated on Abby Care and updated on our mutual patients. A true builder, you’re energized by being part of a founding team, you're not afraid to roll up your sleeves - building playbooks and optimizing processes is in your DNA. Highly consultative, you find energy in building and fostering relationships; honesty and follow through are your calling card. You sweat the details, are highly organized, and understand the complexities of the home care landscape. This is a Full-Time hybrid opportunity based in the Phoenix area, with regular travel to build relationships across the market.

What you’ll work on:

- Refine Our Playbook: We have an idea of what works but need you to help pressure test those assumptions and rebuild what could be done differently.
- Build & Own Relationships: Serve as the primary liaison for our health plan partner's case management team, acting as the trusted main point of contact for their leaders, supervisors, and individual case managers.
- Start With Leaders: Map the case management organization, find a champion on each team, and plan how and when we reach their teams. We show up, build the materials and answer questions.
- Promote through Education: Teach who fits our program, how to refer in minutes, and what happens next. Run team sessions and a launch roadshow of in-person events. Lead with listening, never with a pitch.
- Keep Case Managers in the Loop: Acknowledge every referral the same business day, send updates on every family, and get the care plan to the payor case manager when it is done. Be proactive so case managers don’t have to ask for an update.
- Own Authorizations: Request, track, and chase authorizations from submission through approval and renewal. Removing barriers is key.
- Support Providers: Early in the launch, help support PCP office relationships: records, status updates, and stalled requests.
- Track Everything: Log every interaction, referral, and authorization in our CRM, and use it to see where a relationship needs attention. You develop the plan and reprioritize as you see fit.

What you’ll have:

- 5+ years of relationship, care coordination, provider or payor relations, or business development experience, ideally with a health plan, a case management team, or home- and community-based care.
- A feel for how case managers think and what they need to place a family with confidence. Familiarity with the Arizona Medicaid landscape is a big plus.
- A consultative edge. You'll become the go-to person case managers call first, not just another vendor in their inbox.
- Great written and verbal communication. You're comfortable being the face of Abby Care to people more senior than you.
- Organized to a fault. You love a tracker and hate dropping a ball.
- A founding mindset: you have built a relationship, a program, or a process from zero, and you liked it.
