We’re looking for an Advanced Practice Provider (Nurse Practitioner or Physician Associate) to join our virtual team for six months, primarily to provide parental leave coverage, with the potential for the role to transition into a longer-term, full-time position. As an Advanced Practice Provider on our virtual care team, you’ll play a critical role in delivering mostly asynchronous primary care to our members. You’ll care for adult patients across preventive care, chronic disease management, and common acute concerns. What you will do: - Deliver safe, high-quality, patient-centered primary care across asynchronous and synchronous modalities, using strong clinical judgment to triage, manage, and refer patients appropriately. - Build strong, longitudinal patient relationships that drive sustained engagement and high patient satisfaction. - Manage a fast-paced caseload of at least 20 asynchronous cases per day while maintaining excellent clinical standards. - Communicate with clarity and empathy while adhering to and strengthening team communication norms. - Quickly master Sana’s clinical platforms, AI-assisted workflows, and tools, independently troubleshooting issues and optimizing workflows. - Proactively contribute to clinical protocols, workflows, or operations that enhance patient experience, quality, or efficiency. - Collaborate across clinical, product, and operations teams with humility, a low-ego mindset, and a commitment to shared success. - Use data and clinical insight to support proactive panel management, close care gaps, and improve outcomes as Sana’s population health capabilities evolve.
