Occupational Therapist (OT) - Home Health - East Contra Costa

Brentwood • Antioch • Oakley • Pittsburg

$125,000-$160,000 Estimated Annual Base Earnings $59.81-$71.85 Per Hour

Guaranteed Full-Time Pay Company Vehicle Available $5,000 Sign On Bonus

## Responsibilities

- Perform occupational therapy evaluations and reassessments
- Develop individualized treatment plans
- Help patients improve safety, independence, and functional performance within their home environment
- Recommend adaptive equipment and environmental modifications
- Educate patients and caregivers
- Collaborate with physicians and interdisciplinary team members
- Complete documentation accurately and timely

## Qualifications

- Active California Occupational Therapist License
- Valid driver's license
- Home health experience preferred, but not required.
