You'll work across the client, server, and agent layers, understanding how they fit together end to end: how the frontend talks to the API, how the API queues and tracks work, and how LLM-powered agents process data and return results. This is a strong fit for someone who enjoys working across the stack, is comfortable navigating both traditional web application architecture and newer LLM-agent design patterns, and wants to build tools used directly in pharmaceutical R&D decision-making. ## What You'll Work On - Build and maintain features across our React/TypeScript client and NestJS/TypeScript backend, including UI components, REST APIs, and the data layer connecting them. - Design and extend the client-server architecture: state management, routing, authentication (including SSO/SAML), real-time updates via WebSockets, and background job processing via AWS SQS. - Collaborate on the integration layer between the web app and our LLM-based agents — message schemas, queue-based communication, and file/data handoff. - Work with our conversational AI agent system, which orchestrates multiple specialized agents for statistical modeling, data quality checks, and PK/PD analysis workflows. - Contribute to database schema design and migrations (PostgreSQL/Sequelize), API documentation (Swagger), and test coverage (Jest, pytest). - Participate in code review, architecture discussions, and production support across the stack. ## What We're Looking For - Strong professional experience building production level full stack web applications, comfortable owning features from database to UI. - Strong TypeScript skills across both frontend (React) and backend (Node.js/NestJS) contexts. - Experience designing REST APIs, relational data models (PostGresSQL, MongoDB), and authentication/authorization flows. - Familiarity with message-queue or event-driven architectures (SQS, similar). - Comfort reading and reasoning about systems that integrate with LLMs — understanding agent/tool patterns, prompt-driven workflows, and the practical quirks of working with LLM output, even without deep AI/ML research background. - Strong problem-solving, ownership, and ability to debug across web applications and nondeterministic AI behavior. - Uses Claude Code, Codex, or similar AI coding tools effectively while understanding, validating, and taking responsibility for the resulting code. - Good instincts for reliability and correctness in domains where data quality matters — you ask "what happens when this fails or the data is malformed." - Clear communicator, comfortable working across an evolving codebase with detailed technical documentation. - Operate with a high level of independence and cope with ambiguity ## Nice to Have - Experience with LLM agent architecture a plus - Experience with SAML SSO, multi-tenant systems, or healthcare/life-sciences software. ## About InsightRX ## Company Overview - **One-liner**: InsightRX develops an AI-powered precision dosing platform that helps clinicians and life science organizations individualize drug therapy to maximize safety and efficacy. - **Entity Type**: Private (Series A, $10M raised, plus additional debt/loan financing totaling $13.64M) - **Headquarters**: San Francisco, California, USA - **Founded**: 2015 - **Founders**: Sirj Goswami, PhD (Co-Founder & CEO) and two other pharmacometricians ## Core Business - **Primary industry**: Healthcare IT / Precision Medicine / Clinical Decision Support - **Target customers**: B2B — hospitals, health systems, academic medical centers, and life science/pharmaceutical organizations - **Mission**: To maximize therapeutic success through treatment individualization and population-level analysis. Their vision is a future where every approved medication works as intended, with minimal side effects. ## Products & Services - **InsightRX Nova**: A precision dosing platform that uses Bayesian model-informed precision dosing (MIPD) to help clinicians individualize drug therapy at the point of care. It is deployed in over 1,000 hospitals and health systems. - **InsightRX Apollo**: A real-time analytics and population-level reporting tool for pharmacy program directors, enabling them to track dosing patterns and outcomes across their organization. - **InsightRX Gemini**: An automated vancomycin PK model selection tool launched in August 2024, built on an algorithm trained on more than 300,000 treatment courses. - **InsightRX Astra**: A conversational PK/PD analysis and end-to-end workflow automation tool for life sciences, researchers, and pharmacometricians, launched in December 2025. - **InsightRX Continuous Learning**: A patented process that uses real-world data to continuously improve model accuracy over time. ## Market Standing - **Valuation**: Not publicly disclosed - **Total Funding**: $13.64M (including a $10M Series A in October 2019 and additional debt/loan financing) - **Key Metric**: Over 5.6 million patients impacted, with more than 1.9 million doses personalized using the platform. Trusted by over 1,000 hospitals and health systems. - **Notable Investors/Partners**: Not publicly named in available sources, but the company has raised a Series A round and additional financing. - **Growth Signals**: Surpassed 1,000 hospital partners; crossed 1 million patients dosed in March 2024 and 5.6 million patients impacted by May 2026; launched two major new products (Gemini in Aug 2024, Astra in Dec 2025); received CE mark certification in November 2020 for EU deployment; featured in a KLAS Emerging Company Spotlight Report with compelling customer feedback for reducing medication dosing errors. ## Competitive Advantages - **Science-first foundation**: Founded by pharmacometricians, not generalist software engineers, giving the company deep domain expertise in quantitative pharmacology. - **Patented Continuous Learning**: Their platform includes a patented process that uses real-world data to continuously improve model accuracy, creating a data moat that strengthens over time. - **Scale and adoption**: Trusted by over 1,000 hospitals and health systems, making them the most widely adopted precision dosing vendor according to their claims. - **AI grounded in science**: Unlike general-purpose AI, their models are built on pharmacokinetic science, real patient data, and evidence-based protocols, making them suitable for clinical decision-making where accuracy is critical. - **End-to-end solution**: Covers the full spectrum from clinical development (life sciences) to point-of-care (hospitals), creating a unique value proposition. ## Strategic Focus - **Expanding AI capabilities**: Recent launches of Gemini (automated model selection) and Astra (conversational PK/PD analysis) signal a push toward more autonomous, AI-driven workflows. - **Scaling clinical adoption**: With over 1,000 hospitals already using the platform, the focus is on deepening penetration within existing health systems and expanding to new ones. - **Life sciences expansion**: The Astra product targets pharmaceutical and research organizations, opening a new revenue stream beyond hospital clinical decision support. - **Continuous model improvement**: The patented Continuous Learning process is a core strategic differentiator that they are investing in to maintain their competitive edge. ## Why Work Here - **Mission-driven work**: The company is tackling one of medicine's hardest problems — getting the right dose to the right patient — with a clear mission to maximize therapeutic success and reduce adverse drug events. - **Cross-disciplinary team**: Clinicians, data scientists, pharmacometricians, and engineers collaborate closely, offering a unique environment for those interested in the intersection of software and clinical science. - **Intellectual rigor**: The company values "intellectually rigorous" work and "forward-looking" thinking, as stated in their core values. This is not a typical SaaS company — the science is central to everything. - **Culture values**: Self-directed accountability, elevating those around you, engaged and optimistic attitude, and intellectual rigor are their stated principles. - **Work environment**: The careers page emphasizes that "precision medicine demands precision thinking" and that the work is "as rigorous as the science demands." The team is described as diverse and mission-driven. - **Remote/Hybrid/Office**: Not explicitly stated in available sources, but the headquarters is in San Francisco, California. - **Notable perks**: The company highlights that their work is "fulfilling" and that they "collectively contribute using individual strengths." The focus is on deep domain knowledge and genuine commitment to improving patient care. ## Sources 1. [insight-rx.com](https://www.insight-rx.com/) 2.
