PM&R Nurse Practitioner - SNF Setting Cercle Health | Queens, NY

About The Role
Our success depends on having a brilliant, passionate Nurse Practitioner who can bring our vision to life. You’ll be involved in every aspect of the patient journey, from crafting customized rehabilitation plans to implementing our cutting-edge pain management protocols to ensuring smooth transitions of care home. Without you, we can't create the meaningful, lasting impact that drives everything we do. You will be our first Nurse Practitioner. You'll work closely with our Physiatrists, gradually developing the skills to practice independently as you meet necessary criteria. You are a key player in ensuring that we not only deliver high-quality care but also grow and expand to reach more patients in need. As a member of our team, your responsibilities will include:
- Providing specialized Physical Medicine and Rehabilitation (PM&R) services as a rehab consultant to a SNF in our Bayside, NY location.
- Crafting customized rehabilitation plans for new admissions.
- Assessing and helping manage barriers to therapy including joint or neuropathic pain, spasticity, neurogenic conditions, and more.
- Following patients to track their therapy progress and coordinating with PT, OT, and SLP to ensure optimal rehabilitation outcomes.
- Implementing our innovative clinical protocols designed to enhance non-opioid pain management approaches and reduce unnecessary opioid administration.
- Perform joint and trigger point injections as necessary.
- Participating in multidisciplinary meetings and bridging the gap between therapy and internal medicine.
- Working alongside SNF staff, PCPs, and administrators to create a seamless, patient-centered care experience.
- Engage in culture building opportunities and ongoing education including Monthly Didactic Lectures, virtual meetings with your assigned Mentor, and quarterly dinners to meet with your colleagues.
- Contributing to our data-driven approach to fall prevention and improved therapy outcomes.
Who You Are
We're looking for skilled Nurse Practitioners with the following qualifications:

Skills and Qualifications
- Active Nurse Practitioner/APRN License with New York State certification.
- Ideally, 1-2 years of previous SNF experience, 1-2 years of Pain Management experience
- Experience in Psychiatry or a demonstrated interested in Behavioral Health
- Passion for post-acute care and serving the geriatric population
- Driven by clinical excellence, seeking to learn and stay on top of the latest trends and research.
- A desire for continuous improvement and a growth-mindset to embrace change.
- Confidence to function autonomously as you develop in the role.
- Collaborative team player who can work effectively with our Physiatrists, therapy teams, and SNF staff.
- Strong interpersonal skills with the ability to understand the needs of our facility partners and patients.
Bonus Qualifications (not required):
- Familiarity with medical record systems such as Point Click Care
- Fluent in Spanish or experience working with diverse patient populations.
- Experience with underserved communities
- Previous involvement in clinical/QI initiatives in inpatient settings.

- Full-Time W-2 role ($150,000-180,000) or Per-Diem ($600-700/day)
- Monday to Friday, 9 AM - 5 PM

What You’ll Accomplish in Your First Year
In Your First Month: You have built strong relationships with the SNF therapy and nursing teams, developed a clear understanding of our clinical workflows and EMR, and are confidently managing an initial patient panel under mentorship. By Month 3: You are confidently managing an expanding patient caseload with increasing autonomy and are actively contributing your physiatric expertise in multidisciplinary care meetings. By Month 6: You are operating as a fully autonomous practitioner, leading patient care for a full caseload, and are recognized as a key physiatry consultant and a valued partner by the SNF staff. By Year 1: You are a clinical leader within the practice—mentoring new team members, using data to drive measurable improvements in patient outcomes, and helping to shape the future of our clinical protocols.
