Primary Care Advanced Practitioner (FNP or PA-C) (Chinese Fluency Required)

Join an Established Family Medicine Practice Dr. Dawei Zheng’s physician-led family medicine practice has been serving the Sacramento community for over 25 years, providing comprehensive, patient-centered primary care. The practice is known for its personalized approach, strong patient relationships, and focus on preventive care and chronic disease management.

We are seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA-C) to join our collaborative outpatient team. Providers benefit from practicing in a supportive private practice environment with the ability to build meaningful long-term relationships with patients. The clinic offers a full scope of primary care services with convenient on-site diagnostics. Key Responsibilities

  • Provide comprehensive primary care services in an outpatient setting
  • Manage acute and chronic medical conditions
  • Conduct routine office visits, annual wellness visits, and health risk assessments
  • Perform physical exams and deliver preventive care services
  • Prescribe medications and develop evidence-based treatment plans
  • Educate patients on diagnoses, medications, lifestyle changes, and preventive strategies
  • Coordinate care with specialists, diagnostic facilities, and other healthcare providers
  • Engage in population health initiatives, including routine screenings and preventive measures
  • Utilize telemedicine for appropriate follow-up and consultation
  • Maintain accurate and thorough documentation (ICD-10, HCC coding, risk adjustment)
  • Collaborate closely with physicians and the care team to ensure continuity and quality of care

Qualifications

  • Certified Nurse Practitioner (FNP) or Physician Assistant (PA-C)
  • Active, unrestricted California license and DEA registration
  • MSN / DNP (FNP) or Master’s degree from accredited PA program
  • Minimum 2 years of primary care experience preferred (Family Medicine, Internal Medicine, or Geriatrics)
  • Experience caring for Medicare and senior populations preferred
  • Strong understanding of medical documentation, coding, and risk adjustment
  • EMR experience required (experience with Epic preferred)
  • Medicare enrollment in good standing or eligibility to enroll
  • Fluency in Mandarin or Cantonese required
  • Strong clinical judgment, communication, and teamwork skills
  • Tech-savvy and adaptable in a dynamic clinical environment
